Our mission is to advocate for and support the health and well-being of our community through comprehensive, multidisciplinary assessments and referrals.
Location: Edina, MN office
Job Type: Full-time, in-person
Salary: $60,000 - $65,000 per year, commensurate with education and experience
Position Overview: We are seeking a compassionate and knowledgeable Health and Wellness Coach to join our multidisciplinary team. In this role, you will work directly with patients and their families, providing services in person, and via telehealth, while collaborating with Registered Dietitians, Nurses, Mental Health Professionals, and Care coordinators to deliver holistic dietary support and wellness education to our members.
Key Responsibilities:
- Collaborative Care: Work closely with Registered Dietitians, as well as the multidisciplinary team, to integrate nutritional strategies into clients’ overall treatment and referral plans, facilitating communication and continuity of care.
- Program Development: Assist in the development and implementation of nutrition-focused programs and workshops aimed at enhancing the well-being of members and their families.
- Monitoring & Evaluation: Support, motivate, and educate clients on TEAM’s services via one-on-one coaching. Evaluate client knowledge, readiness to learn, and potential for behavioral changes.
- Advocacy: Serve as a nutrition and fitness advocate, promoting the importance of healthy eating habits and lifestyle choices.
Qualifications:
- Bachelor’s degree in related field.
- Training or credentials required to provide Wellness or Lifestyle Coaching.
- 1-2 years of experience in a clinical, community, or EAP setting is preferred.
- Strong understanding of the relationship between nutrition and mental health.
- Excellent communication and interpersonal skills, with the ability to work collaboratively in a team environment.
- Ability to engage and motivate diverse populations
- Bilingual in Spanish/English is highly encouraged
